Raffle Promo Mechanics:

1. This promo runs from April 16, 2025 to June 16, 2025
2. This promo is open to all except the following:
   Employees of PHILIPS Monitor Philippines, its Authorized Partners, Suppliers, and Agencies.
   Immediate family members of the above, including relatives up to the second degree of consanguinity or affinity.

Prize:

With every purchase of participating PHILIPS or EVNIA monitor models during the promo period, customers are eligible to join the e-Raffle and get a chance to win this amazing prize:

Grand Prize: A Pop Mart Labubu Fall in Wild big size.
Giveaway: A The Monsters Fall in Wild Series Bucket Hat Mini Bag gift for every qualified purchase (until supplies last, via registration).

NOTE: If a participant’s name is drawn more than once, they will receive the prize of higher value only.

3. This promotion is not valid in conjunction with other promotions or discounts.
4. To officially join the e-raffle promo, the customer must register by visiting https://evniamonitorspromo.com/ fill out the form with the following details:
     o Full Name (must be the same as on the receipt)
     o Contact Number
     o E-mail address
     o Residential Address
     o Store Name
     o Purchase Date (must be within the promo period)
     o Model Name
     o Quantity purchased (if more than 1 unit)
     o Product Serial Number
     o Upload all the required attachments:
        ▪ Proof of purchase (must be clear and readable)
        ▪ Valid government-issued ID
        ▪ Photo of the product serial number (found at the back or bottom of the monitor)
        ▪ Photo of Company ID and authorization letter* (if applicable)
*If the purchase was made under a Company or Organization, the authorized representative must provide their company ID along with a company authorization letter signed and certified by the company’s signatory. This letter should authorize them to join the raffle and claim the prize on behalf of the company or organization.

5. The number of earned raffle entries may vary depending on the participating model unit purchased. The promo applies to the participating models. See ANNEX B.
6. This promo is valid for all participating PHILIPS monitors purchased from any partner dealers. See ANNEX A.

7. The customer will receive a confirmation email for the successful entry submitted.
8. PHILIPS Monitor Philippines reserves the right to disapprove raffle entries if the customer fails to meet the conditions or provides incomplete requirements.
9. Customers can register until June 16, 2025, 11:59 PM, and shall qualify for the draw.
10. The raffle draw will be held at the MMD office located at Unit 2606, Prestige Tower, Ortigas, Pasig City, on June 26, 2025 at 1 pm.
11. Name drawn will be subject to initial verification by PHILIPS Monitor Philippines and will be announced after.
12. PHILIPS Monitor Philippines has the right not to disclose the winner if security reasons permit.
13. PHILIPS Monitor Philippines reserves the right to suspend the release of prize or void any raffle entry if misrepresentation or fraud is proven.
14. Winners will be notified via phone call and registered mail for final verification and
details of the prize redemption and their names will be posted at the official Philips Facebook Page.
15. The prizes are not convertible to cash but transferrable.

16. All Labubu giveaways will be dispatched within 15 working days upon successful registration and validation. Customers will be contacted by one of the staff under PHILIPS Monitor to confirm the delivery details.
17. For the Grand Prize, it must be claimed at the MMD office located at Unit 2606, Prestige Tower, Ortigas, Pasig City. From Monday to Friday at 10 am until 5 pm. If the winners reside outside Metro Manila, the Grand Prize will be delivered free of charge within 60 days of receiving the official notice via registered mail and/or email to the customer’s registered address.
     • Upon claiming, the winner must present the email notification from PHILIPS Monitor Philippines along with two (2) original government-issued IDs (with        picture).
    • If the winner is unable to claim personally, the authorized representative must present the following:

       o Signed authorization letter from the winner to claim the prize on their behalf.                                                                                                                                  o Two (2) original government-issued IDs (with picture and signature) of the winner.
        o Two (2) original government-issued IDs (with picture) of the authorized representative.

• If the winner would like to transfer the prize to a new recipient, regardless of the relationship, the original winner is responsible for informing PHILIPS Monitor Philippines via email marketing@mmdsg.com. 

• The new recipient must provide:
     o Signed certification letter from the original prize winner stating the transfer of the prize.
     o Two (2) original government-issued IDs (with picture and signature) of the original winner.
     o Two (2) original government-issued IDs (with picture) of the new recipient.

19. PHILIPS Monitor Philippines staff will contact the grand prize winner to confirm details.

By accepting the prize, the winner agrees to hold PHILIPS Monitor Philippines and its promo agency free and harmless from any loss, expense, cost, claims, injury, or damage arising from their participation in the program or the acceptance and/or use of the prize.

20. PHILIPS Monitor Philippines reserves the right to disclose and publish the name, photos, and other information of the winner in any mode or manner they deem appropriate.
